Please fill in all sections of this form and put N/A if that section is not applicable. The form must be received by Wednesday evening prior to the first weekend when notice is to be published. Normally, you should receive at least an email acknowledgement within 48 hours of receipt of the form but the office can be busy on certain days and times of the year.
